What is the consultation form builder?

Covered Beauty’s consultation form builder is a free tool that makes a printable client consultation form for a beauty treatment. Pick the treatment, from nails and lashes to injectables and sunbeds, tick the questions you want, add your business name, then print it or save it as a PDF. There’s nothing to sign up for, and nothing you type is saved.

We make Covered Beauty, a booking system for UK beauty businesses, and the builder uses the same forms our salons start from. Nothing here is legal or medical advice. Your insurer’s rules and the product manufacturer’s instructions come first.

Where do the questions come from?

We haven’t written new questions for the builder. Each one comes from our ready-made forms or our guides:

  • Questions for the treatment are word for word from the ready-made forms in Covered Beauty, which has 23 in all.
  • General health questions come from our new client medical history form, leaving out any the treatment’s form already asks.
  • Questions you might add are the ones our guide to that treatment’s form suggests, starting unticked. The guide says why each one is worth asking.

One note is worded differently. On the injectables form, the builder’s under-18 note reads “We can’t do this treatment for anyone under 18.” The law we quote below covers England, and that line is true wherever you work.

Read what to ask and why for:

What goes on every form?

  • Who the client is. Their full name, date of birth and home address, so the signed form stands on its own as a record.
  • Their explicit consent. Health information is special category data under UK GDPR, so every form ends with a separate tick for the client’s explicit consent to you using it. What that means for you is in our guide to consent forms and UK GDPR.
  • Signatures. Lines for the client’s signature and yours, each with the date, and their name in capitals.
  • A patch test record when the form asks about a patch test: the date, the product and batch number, where it was applied, the result and who did it. Add or take it off as you like.

Our blank forms to print end the same way. They’re on free salon consultation form templates, if you’d rather download a ready-made one as it is.

Botox, fillers and the under-18 law

In England, the Botulinum Toxin and Cosmetic Fillers (Children) Act 2021 makes it an offence to give anyone under 18 botulinum toxin (such as Botox), or a filler injection for a cosmetic purpose. It gives a defence to a registered doctor, and to a regulated health professional acting on a doctor’s directions. The injectables form asks every client whether they’re under 18. If you work in Scotland, Wales or Northern Ireland, check the rules there with your insurer.

Injectables and electrolysis carry more risk than most treatments, so the builder asks you to have those forms checked against your insurer’s requirements and your professional body’s guidance before you use them.

Paper or online?

Paper works, but the client fills it in once they’re already in your chair, and the forms need locking away. Covered Beauty sends the form with the booking instead:

  • With their booking. Link a form to your treatments and it goes out with the confirmation, with a reminder 48 hours before if it isn’t done.
  • A private link. Email it, or copy it into WhatsApp or a text. It works for 14 days and only for that client.
  • On your phone or tablet. Set a kiosk PIN, then use Hand to client. It shows only their forms until someone from your team enters the PIN.

Every signed form lands on the client’s record, ready to print or download as a PDF. Try Covered Beauty free. No card needed for your free week, and forms are on both plans, from £9 a month.

Questions about the form builder

Is the consultation form builder free?

Yes. There’s no account to make and nothing to pay. Build a form for every treatment you do, and print them or save them as PDFs as often as you like.

Do you keep what I type into the builder?

No. The builder works in your browser. Nothing you choose or type in it is sent to us or saved, and closing the page clears it.

Are the forms approved by insurers?

No. They’re starting points, not approved by any insurer or checked by a lawyer. Read the form through, add anything your treatments need, and check it suits your insurance and your training before you use it. If your insurer gives you approved wording, use that.

Which treatments can I make a form for?

Nails, lash extensions, lash lifts, lash tints, brow lamination, brow tints, brow henna, brow waxing, hair removal and waxing, facials and peels, LED light therapy, injectables, spray tans, sunbeds, massage, electrolysis, hair extensions and dermaplaning. For anything else, start from the closest one and add your own questions.

How do I save the form as a PDF?

Press Print or save as PDF. In the print window, choose Save as PDF where you’d choose a printer, if your phone or computer offers it. The page prints only the form, on A4.

Can clients fill in the form online instead?

Yes, with Covered Beauty. Our ready-made forms are in it, ready to edit. Link a form to your treatments and it goes out with the booking confirmation, with a reminder 48 hours before if it isn’t done. Clients sign on their phone and the signed form lands on their record. It’s free for a week, with no card needed, then from £9 a month.
